Option 1. We can help!
Please book your preferred appointment and we will do our best to help you request a valid referral if needed.
Please visit BOOK NOW and select a Subsidize Rate Requested consult. Once booked watch your email for our Intake and Consent form, then fill it out ASAP so we have all the info required to help you request a referral.
During business hours we will fax a referral form to your health care provider.
Please call their office, or go to their office, and request the referral.
Option 2. DIY!
Have a completed referral form (see below) addressed to the Ontario Breastfeeding Clinic for both you and baby from your Midwife, Family Dr, Nurse Practitioner, or your local Walk-in Dr?

or fax 1-833-615-1113.

Below is a copy of a printable pdf or online referral form that you can complete with:
- your name, date of birth, healthcard number and version code
- your contact information
- baby's name, date of birth, health card number and version code (if available)
If your referring provider uses the online referral form it will be received immediately once submitted.
